Myofascial (Jaw) Pain: The Pain Nobody Talks About

A randomized clinical trial tested oral CBD for jaw pain — and the results are worth knowing.

Most people have never heard the words “myofascial pain disorder of the temporomandibular region.” But millions of them live with it every day. It’s the jaw that aches by noon. The temples that throb after a stressful meeting. The click and catch when you open your mouth too wide. The headaches that seem to come from nowhere but somehow always end at your jaw.

TMD — temporomandibular disorder — affects an estimated 5 to 12% of the population, and myofascial pain (muscle-based jaw pain) is its most common form. Treatment options have always been a patchwork: night guards, physical therapy, anti-inflammatories, muscle relaxants, and sometimes Botox. None of them works reliably for everyone.

A 2026 study published in Medical Cannabis and Cannabinoids tried something different: a placebo-controlled randomized clinical trial of oral CBD for myofascial pain disorder of the temporomandibular region.

What did this trial do?

This was a proper randomized, placebo-controlled clinical trial — not a survey or a case report.

Here’s how it worked:

  • Patients with diagnosed myofascial pain disorder of the temporomandibular region were enrolled.
  • They were randomized to receive oral CBD or a placebo over the study period.
  • Outcomes were assessed using validated pain measures, including:
    • Pain intensity scores
    • Jaw function and range of motion
    • Quality of life and daily impact assessments
    • Any adverse effects

The researchers wanted to answer a specific question: Does oral CBD provide pain relief and functional improvement for people with this type of jaw and muscle pain, compared to doing nothing active?

What did they find?

The results supported CBD as a potentially useful option:

  • Participants in the CBD group reported lower pain intensity scores than the placebo group.
  • Jaw function improved — measures of mouth opening and pain on movement showed better outcomes in the CBD group.
  • Quality of life scores improved in the CBD group, reflecting less daily disruption from pain.
  • CBD was well tolerated, with no serious adverse events reported during the trial.

The authors concluded that oral CBD shows promise as a treatment for myofascial pain disorder of the temporomandibular region, and called for larger trials to confirm the findings and establish optimal dosing.

Why might CBD help with jaw and muscle pain?

Myofascial pain in the jaw involves a combination of muscle tension, inflammation, and central pain sensitization — meaning the nervous system becomes over-responsive to pain signals over time.

CBD’s potential mechanisms for this type of pain include:

  • Anti-inflammatory effects that may reduce localized muscle and joint inflammation
  • Muscle relaxation through interaction with receptors involved in muscle tone
  • Central pain modulation — CBD may help calm an overstimulated pain response in the nervous system
  • Anxiety reduction — since stress is a major driver of jaw clenching and myofascial tension, CBD’s anxiolytic properties may indirectly reduce pain

What this means for you

The problem: jaw and muscle pain from TMD is common, often chronic, and frequently undertreated — partly because it sits at the intersection of dentistry, neurology, and pain medicine, and patients can fall through the cracks.

Here’s what to take from this research:

  • CBD is being studied seriously for a condition that has very few well-studied treatment options beyond physical therapy and bite appliances.
  • Oral CBD is accessible — unlike injectable formulations or prescription medications, CBD oil is available without a prescription in most US states.
  • Work with your dentist or pain specialist. TMD treatment works best as a team effort. If you’re interested in trying CBD as an add-on, bring this study to your next appointment and discuss it as part of a broader plan.
  • Stress management remains essential. If stress and anxiety are driving your jaw pain — and for most people with myofascial TMD, they are — CBD’s anxiolytic effects may help more than its direct analgesic ones.
